Management begins with comprehensive lifestyle modification: weight reduction in overweight individuals, regular aerobic exercise (≥150 minutes per week), strict limitation of refined carbohydrates and added sugars (especially fructose), avoidance of alcohol, and replacement of saturated and trans fats with unsaturated fats. For patients with very high triglycerides (>500 mg/dL), pharmacologic intervention—such as prescription omega-3 fatty acid formulations (e.g., icosapent ethyl), fibrates (e.g., fenofibrate), or niacin (less commonly used today due to safety concerns)—may be indicated alongside lifestyle changes to prevent pancreatitis. Statins are often added if concomitant LDL cholesterol elevation or established cardiovascular disease is present.
Regular monitoring—including fasting lipid panels, glycemic assessment (HbA1c, fasting glucose), and thyroid function tests—is essential to identify and address contributing factors. Patients should be counseled that sustained triglyceride control not only reduces acute risks like pancreatitis but also supports long-term cardiovascular resilience and metabolic health.
