What are the functions and roles of the prostate gland?
The prostate is a small, walnut-sized gland located in the male pelvis, situated just below the bladder and surrounding the proximal portion of the urethra. Its primary physiological functions are both reproductive and supportive of urinary function. Structurally, it consists of glandular and stromal tissue, with ducts that open into the prostatic urethra.
One of its key roles is the production and secretion of prostatic fluid—a milky, alkaline component that constitutes approximately 25–30% of the total seminal volume. This fluid contains critical substances including prostate-specific antigen (PSA), which liquefies coagulated semen after ejaculation to enhance sperm motility; zinc, which supports sperm viability and has antimicrobial properties; citric acid, which provides energy for sperm metabolism; and acid phosphatase, an enzyme involved in sperm function. By contributing to seminal plasma composition, the prostate directly supports fertility.
In addition to its reproductive role, the prostate plays an important part in urinary continence and voiding. Its smooth muscle fibers and anatomical position around the urethra contribute to the internal urethral sphincter mechanism, helping maintain urinary control at rest. During ejaculation, coordinated contraction of prostatic smooth muscle expels secretions into the urethra, while simultaneous relaxation of the bladder neck prevents retrograde ejaculation.
It is important to note that the prostate undergoes age-related changes—most notably benign prostatic hyperplasia (BPH)—which can lead to lower urinary tract symptoms (LUTS) such as urinary frequency, urgency, weak stream, or nocturia. Conversely, abnormalities such as prostatitis (inflammation), prostatic intraepithelial neoplasia (PIN), or prostate cancer may impair normal function and require clinical evaluation. Regular screening—including digital rectal examination (DRE) and serum PSA testing—may be indicated based on age, risk factors, and shared decision-making with a healthcare provider.
