If lochia has completely resolved—meaning no bleeding or discharge for at least several days—and then fresh, bright red blood reappears, this is termed “rebleeding” or “secondary postpartum hemorrhage.” This is not normal and warrants prompt medical evaluation.
Rebleeding after lochia has ceased may indicate an underlying issue such as retained placental tissue or clots within the uterine cavity, endometritis (infection of the uterine lining), uterine atony (inadequate uterine contraction), cervical or vaginal lacerations that reopened, or, less commonly, gestational trophoblastic disease or coagulopathy. Risk factors include prolonged labor, chorioamnionitis, manual placental removal, or prior uterine surgery.
Any recurrent vaginal bleeding—especially if accompanied by fever, foul-smelling discharge, pelvic pain, tachycardia, dizziness, or signs of hemodynamic instability—requires urgent assessment. Clinical evaluation typically includes pelvic examination, transvaginal ultrasound to assess for retained products or uterine abnormalities, and laboratory testing (e.g., CBC, CRP, blood cultures if infection is suspected). Management depends on the cause and may involve antibiotics, uterotonic agents, uterine evacuation (e.g., dilation and curettage), or, in rare cases, surgical intervention.
In summary: while intermittent spotting in the early postpartum period can be common, true cessation followed by new-onset bright red bleeding is clinically significant and should never be dismissed as “normal.” Prompt consultation with an obstetrician or healthcare provider is essential to rule out serious complications and ensure timely, appropriate care.
