How is neuropathic pain treated?
Neuropathic pain arises from damage or dysfunction of the peripheral or central nervous system—not from ongoing tissue injury. Effective management requires a multimodal approach tailored to the underlying cause, symptom severity, and individual patient factors.
First-line pharmacologic treatments include anticonvulsants such as gabapentin and pregabalin, which modulate calcium channels to reduce neuronal hyperexcitability; and certain antidepressants—particularly tricyclic antidepressants (e.g., amitriptyline, nortriptyline) and serotonin–norepinephrine reuptake inhibitors (e.g., duloxetine, venlafaxine)—which enhance descending inhibitory pain pathways. These agents are supported by robust clinical trial evidence and are recommended in major guidelines, including those from the International Association for the Study of Pain (IASP) and the American Academy of Neurology.
Second-line options may include topical agents like high-concentration capsaicin patches or lidocaine 5% patches—especially useful for localized neuropathic pain with intact skin—and tramadol, a weak mu-opioid agonist with SNRI activity. Opioids are generally discouraged for chronic neuropathic pain due to limited efficacy, significant risks of tolerance, dependence, and adverse effects, and lack of long-term benefit per current evidence.
Nonpharmacologic strategies play a critical adjunctive role: structured physical therapy can improve function and reduce maladaptive movement patterns; cognitive behavioral therapy (CBT) helps patients develop coping skills and address pain-related distress and catastrophizing; and neuromodulation techniques—including spinal cord stimulation (SCS) or peripheral nerve stimulation—may be considered for refractory cases after thorough multidisciplinary evaluation.
Underlying etiologies must always be addressed: optimizing glycemic control in diabetic neuropathy, treating active infections (e.g., shingles with antivirals), managing autoimmune conditions, or correcting nutritional deficiencies (e.g., B12 deficiency). A comprehensive assessment—including detailed history, neurological examination, and targeted investigations (e.g., nerve conduction studies, MRI, serologic testing)—is essential before initiating therapy.
Treatment should be initiated at low doses, titrated gradually to balance efficacy and tolerability, and regularly reassessed using validated tools (e.g., DN4, PainDETECT, or numeric rating scale). Shared decision-making, realistic goal setting (e.g., 30–50% pain reduction and improved function rather than complete elimination), and ongoing monitoring for adverse effects and treatment adherence are fundamental to successful long-term management.
