Yes, a plantar wart adjacent to an open wound significantly increases the risk of transmission. Plantar warts are caused by certain strains of human papillomavirus (HPV), most commonly HPV types 1, 2, 4, 27, and 58. The virus gains entry through microtraumas or breaks in the skin—such as cuts, abrasions, or fissures—and replicates within keratinocytes in the epidermis. When a wart is located next to an active wound, the disrupted epidermal barrier provides a direct portal for viral inoculation into deeper layers of uninfected skin. This not only raises the likelihood of autoinoculation (spreading to other areas on the same individual) but also increases the potential for heteroinoculation (transmission to others), especially in shared environments like locker rooms, showers, or swimming pools where barefoot contact with contaminated surfaces occurs. It is therefore critical to keep the area clean, covered with a waterproof dressing, avoid picking or trimming the lesion, and practice strict hand hygiene after touching the site. Prompt evaluation by a dermatologist or podiatrist is recommended to initiate appropriate treatment—such as salicylic acid, cryotherapy, or immunomodulatory agents—and reduce both local spread and transmission risk.
