Epistaxis—commonly known as a nosebleed—is a frequent clinical presentation, particularly during periods of low ambient humidity. While air conditioning itself does not directly cause epistaxis, its operation significantly reduces indoor relative humidity and cools the nasal mucosa, creating conditions that predispose individuals to mucosal drying, microfissuring, and subsequent capillary rupture.
The nasal vestibule and Kiesselbach’s plexus—an anastomotic network of small arteries located in the anterior inferior septum—are especially vulnerable. Prolonged exposure to dry, cool air from air conditioning units compromises the integrity of the mucociliary barrier, diminishes local blood flow regulation, and impairs epithelial repair mechanisms. This effect is often compounded by habitual nose-picking, forceful sneezing or blowing, or underlying comorbidities such as hypertension, chronic rhinitis, or coagulopathy.
Clinically, most air-conditioning–associated epistaxis episodes are anterior and self-limited, resolving within 10–15 minutes with proper first-aid measures—including bilateral digital compression, upright positioning with slight forward flexion, and avoidance of recumbency or posterior nasal packing unless indicated. Persistent or recurrent bleeding warrants evaluation for structural abnormalities (e.g., septal deviation), systemic disorders, or medication-related effects (e.g., anticoagulants, NSAIDs).
Preventive strategies include maintaining indoor relative humidity between 40% and 60% using humidifiers, applying saline nasal gel or ointment to the anterior nares before sleep or prolonged AC exposure, and ensuring adequate hydration. Patients with recurrent epistaxis should undergo targeted otolaryngologic assessment to exclude treatable etiologies and optimize long-term mucosal health.
