This presentation commonly suggests involvement of the lumbar spine, particularly nerve root irritation or compression—most frequently at the L4–L5 or L5–S1 levels.
One of the most common underlying causes is lumbar radiculopathy, often due to a herniated intervertebral disc. When disc material protrudes or extrudes into the spinal canal, it may impinge on the emerging right L5 or S1 nerve root, producing sharp, shooting, or burning pain along the dermatomal distribution—typically extending from the posterior iliac crest down the posterior-lateral thigh, calf, and sometimes into the foot or toes. Pain intensifies with maneuvers that increase intradiscal pressure, such as Valsalva (e.g., coughing, straining), forward bending, or resisted leg extension.
Other potential etiologies include lumbar spinal stenosis—especially neurogenic claudication—where walking or standing triggers bilateral or unilateral leg discomfort, heaviness, or cramping that improves with sitting or flexion. Less common but important considerations include spondylolisthesis, facet joint arthropathy, sacroiliac joint dysfunction, or, rarely, space-occupying lesions such as tumors or infections affecting the cauda equina or nerve roots.
Red flag symptoms requiring urgent assessment include progressive neurological deficits (e.g., foot drop, saddle anesthesia), bowel or bladder dysfunction (urinary retention or incontinence), or unexplained systemic signs like fever or unintentional weight loss. These may indicate cauda equina syndrome or serious spinal pathology necessitating immediate imaging and neurosurgical consultation.
Initial evaluation should include a detailed history focusing on onset, duration, aggravating and relieving factors, associated neurological symptoms, and functional impact. Physical examination must assess gait, lumbar range of motion, straight leg raise test (positive if reproduction of radicular pain below the knee), motor strength (especially dorsiflexion and plantarflexion), sensory testing, and reflexes (patellar and Achilles). Magnetic resonance imaging (MRI) of the lumbar spine remains the gold standard for visualizing soft-tissue pathology, including disc herniation, neural compression, and spinal canal narrowing.
Management is typically conservative initially—comprising activity modification, nonsteroidal anti-inflammatory drugs (NSAIDs), physical therapy emphasizing core stabilization and neural mobilization, and possibly short-term oral corticosteroids. Epidural steroid injections may be considered for persistent radicular pain refractory to conservative measures. Surgical intervention—such as microdiscectomy or decompressive laminectomy—is reserved for patients with progressive neurological deficits, intractable pain, or cauda equina syndrome.
