Who Should Avoid Eyelash Extensions?

While eyelash extensions have become increasingly popular for enhancing natural lash appearance, they are not suitable for everyone. Certain medical conditions and ocular health factors can significantly increase the risk of complications, making the procedure inadvisable.

Individuals with active ocular surface disease—including blepharitis, meibomian gland dysfunction, or chronic dry eye syndrome—should avoid eyelash extensions. These conditions compromise the integrity of the tear film and lid margin, and adding foreign material to the lashes can exacerbate inflammation, impair blink mechanics, and worsen tear evaporation.

Patients with a history of allergic conjunctivitis or known sensitivities to acrylate-based adhesives—commonly used in extension application—are at heightened risk for allergic or irritant contact dermatitis of the eyelids and periocular skin. Similarly, those with atopic dermatitis or rosacea involving the periocular region may experience flare-ups triggered by adhesive exposure or mechanical irritation.

Recent ocular surgery—such as LASIK, cataract extraction, or ptosis repair—constitutes a temporary contraindication. Extensions should be deferred until the eye has fully healed and postoperative inflammation has resolved, typically for at least four to six weeks, depending on surgical complexity and individual recovery.

Additionally, individuals undergoing active treatment with topical ophthalmic medications (e.g., corticosteroids, antibiotics, or glaucoma agents) or systemic immunosuppressants may exhibit delayed wound healing or altered immune responses, increasing susceptibility to infection or adverse reactions.

Clinicians advise thorough pre-procedure assessment—including slit-lamp examination when indicated—to identify contraindications. Patients should disclose all ocular and systemic health conditions, current medications, and prior adverse reactions to cosmetic procedures before considering eyelash extensions.
