However, certain populations should avoid it entirely due to well-documented physiological and clinical risks.
Individuals with active liver disease—including alcoholic hepatitis, cirrhosis, or non-alcoholic fatty liver disease (NAFLD)—must abstain from yellow rice wine. Even modest alcohol intake can exacerbate hepatic inflammation, accelerate fibrosis progression, and impair detoxification capacity. Similarly, patients with chronic pancreatitis or a history of alcohol-induced pancreatitis are at heightened risk for recurrent attacks, as ethanol metabolites directly stimulate pancreatic acinar cell injury and inflammatory cascades.
People diagnosed with uncontrolled hypertension or heart failure should also refrain from consumption. Alcohol induces transient sympathetic activation and vasodilation followed by rebound vasoconstriction, potentially destabilizing blood pressure control and increasing cardiac workload. In heart failure, even low-dose ethanol may worsen ventricular remodeling and reduce ejection fraction over time.
Patients taking medications metabolized by cytochrome P450 2E1 (CYP2E1)—such as acetaminophen, isoniazid, or certain anticonvulsants—are at increased risk of hepatotoxicity when consuming yellow rice wine. Ethanol competitively inhibits CYP2E1 at low doses but induces it chronically, leading to unpredictable drug clearance and accumulation of toxic intermediates.
Additionally, individuals with gout or hyperuricemia should avoid yellow rice wine due to its purine content and ethanol-induced uric acid elevation. Alcohol metabolism increases lactate production, which competes with uric acid for renal excretion, thereby raising serum urate levels and triggering acute gout flares.
Finally, yellow rice wine is contraindicated in pregnancy and lactation. No safe threshold of alcohol exposure has been established for fetal neurodevelopment; prenatal ethanol exposure remains a leading preventable cause of congenital neurobehavioral disorders, including fetal alcohol spectrum disorders (FASD). During lactation, ethanol rapidly equilibrates in breast milk, potentially affecting infant sleep architecture and motor development.
