Dark circles are not a single condition but rather a heterogeneous clinical sign with multiple potential etiologies, including dermal melanin deposition, thinning of the infraorbital skin leading to visible vasculature, shadowing from lower eyelid fat prolapse or tear trough deformity, and post-inflammatory hyperpigmentation.
Prior to treatment, a board-certified dermatologist or oculoplastic surgeon will assess skin type, thickness, vascularity, volume loss, and bony anatomy using clinical examination and, when indicated, high-resolution imaging. Patients must disclose all current medications—including anticoagulants, NSAIDs, and herbal supplements—as well as a history of keloid formation, bleeding disorders, or active dermatoses such as atopic dermatitis or rosacea in the periorbital region.
Preoperative counseling is essential: patients should be informed that no single modality universally resolves dark circles, and realistic expectations must be established. Topical agents (e.g., hydroquinone, retinoids, vitamin C), energy-based devices (e.g., low-fluence Q-switched lasers, non-ablative fractional resurfacing), injectable fillers (e.g., hyaluronic acid for tear trough correction), or surgical interventions (e.g., transconjunctival fat repositioning) may be recommended—depending on the dominant pathophysiology. Sun protection with broad-spectrum SPF 30+ is mandatory both before and after treatment to prevent pigmentary rebound.
Patients are typically advised to discontinue photosensitizing medications and supplements one to two weeks preoperatively, avoid alcohol for 72 hours prior, and refrain from applying topical retinoids or exfoliants for five days before laser- or device-based therapies.
