Lower eyelid ectropion correction is a well-established surgical procedure designed to restore the normal anatomical position and function of the lower eyelid when it turns outward—away from the eye. This condition, known as ectropion, commonly results from age-related tissue laxity, facial nerve palsy, scarring (e.g., from trauma or prior surgery), or chronic inflammation. Left untreated, ectropion can lead to persistent ocular surface exposure, excessive tearing (epiphora), conjunctival irritation, corneal abrasions, and, in severe cases, vision-threatening keratopathy.
The surgical approach is tailored to the underlying cause. For involutional ectropion—the most common type in older adults—procedures typically involve tightening the lateral canthal tendon and reinforcing the lower eyelid retractors, often combined with a lateral tarsal strip or canthoplasty. In cases secondary to facial nerve dysfunction, a static sling or dynamic reanimation may be considered. Cicatricial ectropion usually requires release of scar tissue and replacement with a skin graft or local flap to restore lid contour and tension.
Outcomes are generally excellent, with high rates of functional and cosmetic success when performed by an experienced oculoplastic or facial plastic surgeon. Most patients experience rapid symptom relief—reduced tearing, improved comfort, and protection of the ocular surface—within days to weeks postoperatively. Complications are uncommon but may include transient bruising, swelling, mild asymmetry, or temporary dryness; serious adverse events such as infection or recurrence are rare with appropriate patient selection and technique.
Patient counseling should emphasize realistic expectations, preoperative assessment—including evaluation of tear film stability, orbicularis muscle tone, and horizontal lid laxity—and adherence to postoperative care instructions to optimize healing and long-term results.
