Chronic or recurrent headaches triggered by poor sleep—often described as “tension-type” or “hypnic” in nature—are a common yet underrecognized clinical concern. Sleep disturbances, including insufficient duration, fragmented architecture, or circadian misalignment, can lower pain thresholds, dysregulate hypothalamic-pituitary-adrenal axis activity, and amplify cortical excitability—all of which contribute to headache susceptibility.
Effective management begins with a thorough assessment: clinicians should evaluate sleep hygiene practices, screen for comorbid conditions such as insomnia, obstructive sleep apnea, or restless legs syndrome, and rule out secondary causes like medication overuse or intracranial pathology. Polysomnography may be warranted if sleep-disordered breathing is suspected.
First-line interventions emphasize behavioral modification. Patients benefit from consistent sleep-wake schedules—even on weekends—avoidance of caffeine and blue-light exposure within three hours of bedtime, and cognitive behavioral therapy for insomnia (CBT-I), which has demonstrated robust efficacy in reducing both sleep latency and headache frequency in randomized trials.
Pharmacologic approaches are reserved for refractory cases and must be individualized. Melatonin supplementation (0.5–3 mg) taken 30–60 minutes before bedtime may improve sleep onset and continuity, particularly in circadian rhythm disorders. For patients with comorbid migraine, preventive agents such as low-dose tricyclic antidepressants (e.g., amitriptyline 10–25 mg at night) may serve dual purposes—modulating sleep architecture while reducing headache burden.
Importantly, acute analgesics—including NSAIDs or triptans—should not be used more than two to three days per week to avoid medication-overuse headache. A multidisciplinary approach—integrating neurology, sleep medicine, and behavioral health—is often optimal for sustained improvement.
