In men, clinically significant low testosterone (hypogonadism) often manifests as diminished libido, erectile dysfunction, reduced spontaneous erections, fatigue, decreased muscle strength and lean body mass, increased adiposity—particularly central fat accumulation—and mood changes such as irritability, low motivation, or depressive symptoms. Some individuals also report sleep disturbances, reduced bone mineral density, and diminished cognitive sharpness, including difficulties with concentration and memory.
In women, low estrogen—commonly seen during perimenopause, postmenopause, or due to hypothalamic amenorrhea, premature ovarian insufficiency, or pituitary disorders—may present with vasomotor symptoms (e.g., hot flashes and night sweats), vaginal dryness and atrophy, dyspareunia, irregular or absent menstrual cycles, decreased bone density, and heightened risk of osteoporosis. Mood lability, anxiety, difficulty sleeping, and reduced sexual desire are also frequently reported. Low progesterone, often occurring alongside estrogen fluctuations, may contribute to menstrual irregularities, heavy or prolonged bleeding, and premenstrual emotional symptoms.
It is important to note that symptoms alone are insufficient for diagnosis. Confirmation requires targeted blood testing—ideally drawn in the morning for testosterone (due to diurnal variation) and timed appropriately across the menstrual cycle for estrogen and progesterone—alongside clinical evaluation to rule out secondary causes such as pituitary tumors, chronic illness, obesity, or medication effects. Management is individualized and may include hormone replacement therapy, lifestyle interventions, or treatment of underlying endocrine or systemic conditions.
