Treatment is never directed solely at lowering viscosity with medications; instead, it targets the root cause.
For patients with primary myeloproliferative neoplasms like polycythemia vera, first-line therapy typically includes low-dose aspirin to reduce thrombotic risk and phlebotomy to lower hematocrit and red blood cell mass. In higher-risk individuals—such as those over age 60 or with a history of thrombosis—cytoreductive agents like hydroxyurea or interferon-alpha may be added to control excessive blood cell production.
In cases driven by paraproteinemia (e.g., multiple myeloma or Waldenström macroglobulinemia), plasma exchange (plasmapheresis) can provide rapid, short-term reduction in serum viscosity when symptomatic hyperviscosity syndrome is present—manifesting as headache, blurred vision, retinal hemorrhages, or neurologic deficits. Definitive management, however, relies on disease-specific antimyeloma or lymphoplasmacytic therapies such as bortezomib, rituximab, or ibrutinib.
Importantly, no FDA-approved drugs exist specifically for “thinning” blood based solely on viscosity measurements in otherwise healthy individuals. Over-the-counter supplements like fish oil or nattokinase lack robust clinical evidence for efficacy or safety in this context and should not replace evidence-based care. Clinical evaluation—including complete blood count, peripheral smear, serum protein electrophoresis, and bone marrow assessment when indicated—is essential before any therapeutic intervention.
