Lifestyle and surveillance strategies play a critical role in optimizing outcomes and preserving renal function.
Regular oncologic follow-up is essential. Imaging—typically contrast-enhanced CT or MRI—is recommended every 3–6 months during the first two years post-treatment, then annually for at least five years, depending on tumor stage and risk stratification. Surveillance includes monitoring for local recurrence, distant metastases (particularly to lung, bone, liver, and brain), and synchronous or metachronous contralateral renal tumors.
Hypertension, chronic kidney disease, and metabolic syndrome are prevalent comorbidities in ccRCC patients and must be actively managed. Blood pressure control—ideally below 130/80 mmHg—is crucial, especially in those with reduced renal mass or post-nephrectomy. Angiotensin-converting enzyme inhibitors or angiotensin II receptor blockers are often preferred due to their renoprotective effects.
Nutrition and physical activity significantly influence prognosis. A Mediterranean-style diet—rich in fruits, vegetables, whole grains, lean proteins, and unsaturated fats—is associated with improved survival and lower cardiovascular risk. Patients should avoid tobacco use entirely and limit alcohol consumption. Regular moderate-intensity exercise (e.g., brisk walking ≥150 minutes per week) supports immune function, mitigates treatment-related fatigue, and helps maintain healthy body composition.
Given the strong association between obesity and ccRCC pathogenesis, weight management is a cornerstone of care. Even modest weight loss (5–10% of baseline body weight) can reduce systemic inflammation and insulin resistance—both biologically implicated in tumor progression.
Patients should also be counseled on medication safety: nonsteroidal anti-inflammatory drugs (NSAIDs) should be used cautiously, particularly in those with compromised renal function, and proton pump inhibitors should be prescribed judiciously to minimize potential microbiome and immune modulation effects. Participation in clinical trials—especially for high-risk or recurrent disease—remains an important option aligned with evolving standards of care.
