The collection of hematopoietic stem cells (HSCs), the progenitor cells responsible for generating all blood cell types, is a critical procedure in treating various life-threatening conditions such as leukemia, lymphoma, and certain genetic disorders. The two primary methods employed for HSC harvesting are peripheral blood stem cell (PBSC) donation and bone marrow donation. Each approach has distinct procedural characteristics, patient experiences, and clinical indications.
Peripheral Blood Stem Cell Collection, also known as apheresis, is currently the most common method for autologous and allogeneic transplants. Before collection begins, donors typically undergo a regimen of growth factors, specifically granulocyte-colony stimulating factor (G-CSF), which stimulates the bone marrow to release increased numbers of stem cells into the bloodstream. Once the stem cell count in the peripheral blood reaches sufficient levels, the donor undergoes the apheresis procedure. During this process, blood is drawn from one arm, passed through an automated machine that separates and collects the CD34-positive stem cells, and returns the remaining blood components to the donor via the other arm. This procedure is generally well-tolerated, often completed over several hours, and may require multiple sessions depending on the target cell dose. Common side effects during the mobilization phase include bone pain, headaches, and flu-like symptoms, while the apheresis itself may cause mild tingling due to citrate anticoagulant use, which can be managed with calcium supplementation.
Bone Marrow Harvesting involves a surgical procedure performed under general or spinal anesthesia. Unlike PBSC collection, this method does not require prior administration of G-CSF. The surgeon accesses the posterior iliac crests—the flat bones at the back of the pelvis—and uses specialized needles to aspirate liquid bone marrow mixed with blood. The collected fluid is then filtered and processed to isolate the stem cells. Although it is considered a minor surgical procedure, it carries higher immediate risks compared to apheresis, including potential complications related to anesthesia, bleeding, infection, and post-operative pain at the harvest site. Recovery typically takes longer, with donors often experiencing significant soreness and fatigue for several days to weeks. Despite these drawbacks, bone marrow donation remains the preferred source for certain pediatric recipients or specific disease states where data suggests superior engraftment outcomes.
The choice between PBSC and bone marrow donation depends on multiple factors, including the donor’s health status, the recipient’s medical needs, and institutional protocols. While PBSC offers greater convenience and less invasive recovery, bone marrow provides a different cellular composition that may benefit specific transplant scenarios. Regardless of the method, rigorous screening ensures donor safety, and both procedures are conducted by specialized teams in accredited transplantation centers to maximize efficacy and minimize risks.
