The vestibular system is a sensory apparatus located within the inner ear that detects head position, motion, and spatial orientation. It comprises three semicircular canals—oriented roughly orthogonally to one another—and two otolith organs: the utricle and the saccule. The semicircular canals respond primarily to angular acceleration (e.g., rotational head movements), while the utricle and saccule detect linear acceleration and static head tilt relative to gravity via calcium carbonate crystals (otoconia) embedded in gelatinous membranes overlying hair cells.
This system works in concert with visual input and proprioceptive signals from muscles and joints to generate a coherent perception of balance, posture, and movement. Neural signals from vestibular hair cells travel via the vestibular branch of cranial nerve VIII (the vestibulocochlear nerve) to brainstem nuclei, the cerebellum, and thalamocortical pathways—ultimately contributing to gaze stabilization, postural control, spatial navigation, and autonomic regulation.
Vestibular dysfunction may manifest as vertigo, dizziness, imbalance, nausea, or oscillopsia (the illusion that stationary objects are moving), and can arise from conditions such as vestibular neuritis, Ménière disease, benign paroxysmal positional vertigo (BPPV), labyrinthitis, or central neurological disorders. Accurate diagnosis typically involves clinical assessment—including the Dix-Hallpike maneuver for BPPV—and objective testing such as videonystagmography (VNG), video head impulse testing (vHIT), or vestibular evoked myogenic potentials (VEMP).
