When a child experiences a low-grade fever—typically defined as a temperature between 37.5°C and 38.5°C (99.5°F–101.3°F)—that persists for several days without resolution, it warrants careful clinical evaluation. Unlike high fevers, which often signal acute infection or systemic inflammation, prolonged low-grade fever may reflect subtler underlying processes.
Common infectious causes include viral upper respiratory infections, Epstein-Barr virus (EBV) mononucleosis, cytomegalovirus (CMV), or persistent bacterial infections such as urinary tract infections (UTIs), sinusitis, or otitis media—particularly when symptoms are atypical or mild. In younger children, UTIs may present with fever as the sole manifestation, making urinalysis and urine culture essential in the diagnostic workup.
Non-infectious etiologies must also be considered. These include autoimmune conditions such as juvenile idiopathic arthritis (JIA), where fever may accompany subtle joint swelling or morning stiffness; inflammatory bowel disease (IBD), especially in older children presenting with weight loss or gastrointestinal symptoms; or periodic fever syndromes like PFAPA (periodic fever, aphthous stomatitis, pharyngitis, and cervical adenitis).
Less common but critical diagnoses include malignancies—such as leukemia or lymphoma—where persistent low-grade fever may be accompanied by fatigue, pallor, bruising, lymphadenopathy, or unexplained weight loss. Chronic granulomatous disease or other primary immunodeficiencies may also manifest with recurrent or prolonged febrile episodes due to impaired pathogen clearance.
Clinical assessment should include a thorough history—duration and pattern of fever, associated symptoms (e.g., rash, cough, abdominal pain, night sweats), recent travel or exposures, immunization status—and a comprehensive physical examination focusing on growth parameters, lymph nodes, joints, heart, lungs, abdomen, and skin. Initial laboratory testing often includes complete blood count with differential, inflammatory markers (C-reactive protein, erythrocyte sedimentation rate), basic metabolic panel, urinalysis, and blood cultures if clinically indicated.
Parents should seek prompt medical evaluation if low-grade fever persists beyond five days, worsens, or is accompanied by red-flag symptoms—including lethargy, poor feeding or decreased oral intake, respiratory distress, persistent vomiting, or signs of dehydration. Early recognition and targeted investigation are key to identifying treatable causes and preventing complications.
