Women commonly experience breast ptosis—the medical term for sagging or drooping of the breasts—as they age. This natural anatomical change results from a combination of factors that affect the structural integrity of breast tissue and supporting ligaments.
The primary contributors to breast ptosis include progressive loss of skin elasticity, gradual stretching and weakening of the Cooper’s ligaments (fibrous bands that anchor breast tissue to the chest wall), and changes in glandular and adipose tissue composition. Hormonal fluctuations—particularly those associated with pregnancy, lactation, and menopause—play a significant role by altering collagen synthesis and fat distribution within the breast.
Additional modifiable risk factors include substantial weight fluctuations, chronic smoking (which impairs collagen production and accelerates skin aging), and prolonged absence of well-fitted supportive bras during high-impact physical activity. While genetics strongly influence baseline breast shape and resilience, lifestyle and physiological transitions collectively determine the timing and degree of ptosis.
It is important to note that breast ptosis is not indicative of disease, nor does it correlate with breast cancer risk. Clinical assessment typically involves visual evaluation and measurement of the distance from the sternal notch to the nipple (sternal notch–nipple distance) and from the inframammary fold to the nipple (inframammary fold–nipple distance) to classify severity using standardized grading systems such as the Regnault scale.
