What Bowel Symptoms Can Hemorrhoids Cause?

Hemorrhoids—swollen, inflamed veins in the rectum or around the anus—can produce several characteristic changes in bowel habits and stool appearance. The most common symptom is painless rectal bleeding, typically noticed as bright red blood on toilet paper, dripping into the toilet bowl, or streaking the surface of the stool. This occurs because hemorrhoidal tissue is highly vascular and easily traumatized during defecation.

Patients may also experience a sensation of incomplete evacuation or persistent rectal fullness, especially with internal hemorrhoids that prolapse or become thrombosed. Straining during bowel movements—often due to concurrent constipation or hard stools—can exacerbate both hemorrhoid formation and symptom severity. In some cases, mucus discharge accompanies bowel movements, resulting from irritation of the anal mucosa by protruding hemorrhoidal tissue.

While hemorrhoids do not alter stool consistency directly, associated factors such as low-fiber diets, dehydration, or sedentary lifestyle frequently contribute to constipation, leading to lumpy or hard stools (Bristol Stool Scale types 1–2). Conversely, diarrhea or frequent bowel movements may increase friction and irritation, worsening symptoms like itching, burning, or swelling. It is important to note that any new-onset rectal bleeding, change in bowel habit, or unexplained weight loss warrants prompt clinical evaluation to rule out more serious conditions—including colorectal cancer, inflammatory bowel disease, or infectious colitis.
