Salicylic acid is a beta-hydroxy acid (BHA) widely used in dermatology for its keratolytic, anti-inflammatory, and comedolytic properties. Unlike alpha-hydroxy acids (AHAs), which are water-soluble, salicylic acid is lipid-soluble—allowing it to penetrate deeply into sebaceous follicles and dissolve excess sebum and corneocytes within the pore lining.
Clinically, topical salicylic acid is FDA-approved for treating mild to moderate acne vulgaris, psoriasis, seborrheic dermatitis, and warts. Its mechanism involves inhibiting cyclooxygenase enzymes, reducing prostaglandin synthesis, and disrupting intercellular desmosomal adhesion in the stratum corneum—leading to controlled exfoliation of hyperproliferative or dyskeratotic epidermal cells.
In acne management, concentrations of 0.5% to 2% are commonly formulated in cleansers, toners, and leave-on treatments. At these levels, salicylic acid unclogs pores, reduces inflammatory lesion formation, and improves overall skin texture without significant systemic absorption. Higher concentrations (10–30%) are reserved for in-office chemical peels under dermatologic supervision, primarily for actinic keratoses or verrucae.
While generally well-tolerated, potential adverse effects include transient stinging, dryness, erythema, and contact sensitization—particularly in patients with atopic dermatitis or compromised skin barriers. It is contraindicated in individuals with salicylate allergy, children under 2 years, and those using concurrent oral retinoids or isotretinoin due to theoretical additive irritation risk.
Emerging evidence also supports adjunctive use in melasma and post-inflammatory hyperpigmentation, where its exfoliative action enhances penetration of depigmenting agents like hydroquinone or tranexamic acid. However, robust randomized trials remain limited, and combination therapy should be guided by board-certified dermatologists.
