Insomnia rooted in blood deficiency—a concept central to Traditional Chinese Medicine (TCM)—presents with a distinct constellation of symptoms that reflect insufficient nourishment of the Heart and Shen (the spirit or mind). Unlike insomnia caused by hyperarousal or psychological stress, blood-deficient insomnia is characterized by difficulty falling asleep rather than frequent awakenings or early-morning awakening. Patients often report prolonged sleep latency—lying awake for 30 minutes or more despite feeling physically tired.
Key associated features include palpitations, especially at rest or upon lying down; dizziness or lightheadedness, particularly with positional changes; pale or sallow complexion; brittle nails and dry, lusterless hair; and generalized fatigue that fails to improve with rest. Women may experience light or absent menstrual flow, delayed cycles, or amenorrhea—clinical signs consistent with diminished blood volume and impaired tissue perfusion.
Importantly, individuals with blood deficiency typically lack signs of heat or agitation: they rarely complain of night sweats, irritability, or a sensation of internal heat. Instead, they often feel chilly, have cold extremities, and prefer warm environments—reflecting diminished Yang support due to inadequate blood to anchor and warm the body’s functional systems.
From a TCM diagnostic perspective, this pattern arises when Spleen Qi fails to generate sufficient blood from food essence, or when chronic blood loss, excessive mental strain, or prolonged illness depletes existing reserves. While not directly equivalent to Western diagnoses like iron-deficiency anemia, blood deficiency may overlap clinically with conditions involving hypovolemia, nutritional insufficiency, or autonomic dysregulation—and warrants thorough evaluation to rule out underlying hematologic, endocrine, or metabolic causes.
