How to Use Carboprost Tromethamine Suppositories

Carboprost tromethamine suppositories are a prostaglandin analog used primarily to manage postpartum hemorrhage (PPH) when first-line uterotonic agents—such as oxytocin or ergometrine—have failed or are contraindicated. This medication induces strong, sustained uterine contractions by binding to prostaglandin F2α receptors on myometrial smooth muscle cells.

The suppository is administered rectally, typically at a dose of 1 mg (one 250-mcg tablet, repeated as needed up to a maximum cumulative dose of 2 mg within 2 hours). Prior to insertion, the patient should be positioned laterally or in the Sims’ position to facilitate optimal absorption. The suppository must be inserted deeply into the rectum—beyond the internal sphincter—to minimize expulsion and ensure consistent systemic absorption. Patients should remain recumbent for at least 15–30 minutes post-administration to enhance bioavailability.

Clinical monitoring is essential: blood pressure, heart rate, oxygen saturation, and uterine tone must be assessed before dosing and at frequent intervals thereafter. Common adverse effects include transient nausea, vomiting, diarrhea, fever, and bronchospasm—particularly in patients with asthma or reactive airway disease. Carboprost is contraindicated in individuals with active cardiac, pulmonary, hepatic, or renal disease, as well as in those with known hypersensitivity to prostaglandins.

This agent is not indicated for labor induction or cervical ripening and should only be used in settings equipped for emergency resuscitation and surgical intervention, given its potent uterotonic effect and potential for uterine hyperstimulation or rupture.
