Facial nerve spasm—commonly referred to as hemifacial spasm when it affects one side of the face—is a neurological condition characterized by involuntary, intermittent, and often progressive contractions of the muscles innervated by the facial nerve (Cranial Nerve VII). While patients may initially describe symptoms as “eye twitching,” true hemifacial spasm typically begins with unilateral eyelid twitching (blepharospasm) and can gradually spread to involve the orbicularis oris, platysma, and other ipsilateral facial muscles.
The most common underlying cause is neurovascular compression—typically by a looping artery (e.g., the anterior inferior cerebellar artery or posterior inferior cerebellar artery) at the root exit zone of the facial nerve in the cerebellopontine angle. Less frequent etiologies include facial nerve injury, Bell’s palsy sequelae, tumors (e.g., vestibular schwannoma), or demyelinating disease. Accurate diagnosis requires high-resolution MRI with FIESTA or CISS sequences to visualize neurovascular relationships and exclude structural lesions.
First-line treatment is pharmacologic: oral anticonvulsants such as carbamazepine or oxcarbazepine may provide partial relief in early or mild cases, though evidence for long-term efficacy is limited. Botulinum toxin type A injections into affected facial muscles remain the gold-standard symptomatic therapy, offering significant improvement in over 90% of patients, with effects lasting approximately three to four months per session. Repeat injections are safe and effective with long-term follow-up.
For patients with medically refractory, disabling spasm—or those seeking definitive treatment—microvascular decompression (MVD) is the only curative surgical option. Performed via a retrosigmoid craniotomy, MVD involves identifying the offending vessel, inserting a non-absorbable Teflon pledget between it and the facial nerve root entry zone, and confirming adequate decompression intraoperatively. Long-term success rates exceed 85%, with durable remission in most responders; however, risks include hearing loss, facial weakness, cerebrospinal fluid leak, and, rarely, stroke or death.
Management should be individualized through shared decision-making, weighing symptom burden, disease progression, patient age and comorbidities, and preferences regarding repeated interventions versus surgical risk. Referral to a multidisciplinary team—including neurologists, neuro-ophthalmologists, and neurosurgeons experienced in cranial nerve disorders—is essential for optimal outcomes.
