Diagnostic differentiation relies on clinical history, symptom timing and localization, dietary triggers, bowel pattern changes, and response to interventions such as antacids or prokinetics. Imaging—such as abdominal X-ray or CT—is reserved for cases with red-flag symptoms (e.g., weight loss, hematochezia, persistent vomiting, or acute onset with severe pain) to rule out structural pathology like obstruction, ileus, or malignancy.
Management differs accordingly: gastric distension often responds to behavioral modifications (e.g., mindful eating, avoiding carbonated drinks), prokinetic agents, or treatment of underlying motility disorders. Intestinal distension may require dietary adjustments (e.g., low-FODMAP diet), enzyme supplementation (e.g., lactase), probiotics, or targeted therapies for IBS or chronic constipation.
