Effective management begins with accurate identification of the offending agent. Patients should be encouraged to keep a detailed symptom diary noting timing, location, duration, and potential exposures. When the trigger remains unclear, allergy testing—including skin prick testing or serum-specific IgE assays—may be warranted. Patch testing is particularly useful for diagnosing allergic contact dermatitis.
First-line symptomatic relief centers on reducing inflammation and interrupting the itch-scratch cycle. Topical corticosteroids (e.g., hydrocortisone 1% for mild cases; higher-potency agents under medical supervision for more severe involvement) remain the cornerstone of localized treatment. For widespread or refractory pruritus, second-generation oral antihistamines—such as loratadine, cetirizine, or fexofenadine—are preferred due to their favorable safety profile and minimal sedative effects.
In addition to pharmacotherapy, non-pharmacologic strategies play a critical role: using fragrance-free, hypoallergenic moisturizers to restore epidermal barrier integrity; avoiding hot water and harsh soaps; wearing loose-fitting, breathable fabrics (e.g., cotton); and applying cool compresses to soothe acute flare-ups.
