Management is not one-size-fits-all; it depends on the underlying cause, patient age, reproductive goals, and symptom burden.
First-line evaluation includes serum testing for FSH, LH, estradiol, anti-Müllerian hormone (AMH), and thyroid-stimulating hormone (TSH), along with pelvic ultrasonography to assess antral follicle count and ovarian morphology. Common etiologies include autoimmune oophoritis, genetic disorders (e.g., Fragile X premutation, Turner syndrome mosaicism), iatrogenic causes (chemotherapy, radiation), metabolic conditions such as polycystic ovary syndrome (PCOS), and idiopathic cases.
Hormonal therapy remains the cornerstone for symptomatic management—especially in women experiencing vasomotor symptoms, genitourinary syndrome of menopause (GSM), or bone mineral density loss. For non-fertility-seeking individuals, systemic estrogen-progestogen therapy (EPT) or estrogen-only regimens (in hysterectomized patients) are evidence-based and guideline-recommended. Transdermal estradiol is often preferred over oral formulations due to lower thrombotic risk.
In women desiring fertility, ovulation induction is tailored to diagnosis: clomiphene citrate or letrozole are first-line agents for anovulatory PCOS; gonadotropins may be used cautiously in select cases of hypothalamic amenorrhea or mild ovarian impairment—but are contraindicated in confirmed POI due to extremely low ovarian reserve and negligible response rates. Assisted reproductive technologies, including donor oocytes, represent the only viable fertility option for most with established POI.
No pharmacologic agent has been proven to restore endogenous ovarian function in confirmed POI. Supplements such as DHEA, melatonin, or dehydroepiandrosterone lack robust clinical trial support and are not endorsed by major societies including the Endocrine Society or the American Society for Reproductive Medicine. Lifestyle interventions—including weight optimization in overweight/obese individuals, regular aerobic and resistance exercise, and smoking cessation—have demonstrated measurable improvements in menstrual cyclicity and insulin sensitivity, particularly in PCOS-related dysfunction.
Long-term monitoring is essential: dual-energy X-ray absorptiometry (DXA) scans for bone health, lipid panels, and glycemic screening should be integrated into routine care. Multidisciplinary collaboration—with endocrinologists, reproductive endocrinologists, mental health professionals, and registered dietitians—optimizes holistic outcomes and addresses the psychosocial impact of diagnosis.
