Gastroesophageal reflux disease (GERD) is a common chronic condition characterized by the retrograde movement of gastric contents into the esophagus, often causing heartburn, regurgitation, and nocturnal symptoms. Sleep position significantly influences reflux severity due to gravitational and anatomical factors affecting lower esophageal sphincter (LES) pressure and gastric emptying.
Current evidence strongly supports left-side sleeping—specifically, the left lateral decubitus position—as the optimal posture for reducing nighttime GERD symptoms. When lying on the left side, the stomach lies below the level of the gastroesophageal junction, promoting gravitational containment of gastric acid and minimizing reflux episodes. In contrast, right-side sleeping positions the stomach above the esophagogastric junction, facilitating acid escape into the esophagus and increasing both the frequency and duration of reflux events.
Multiple physiological studies using pH-impedance monitoring have confirmed that patients in the left lateral position experience significantly fewer reflux episodes and shorter total acid exposure time during sleep compared with those sleeping supine or on the right side. Additionally, left-side positioning enhances gastric emptying and reduces transient LES relaxations—key mechanisms underlying nocturnal reflux.
While elevating the head of the bed remains an important adjunctive measure, positional therapy—particularly consistent left lateral decubitus positioning—is increasingly recognized as a low-risk, nonpharmacologic strategy for managing refractory or nocturnal-predominant GERD. Patients should be counseled that adherence to left-side sleeping may require behavioral support, especially if accustomed to other positions, but offers measurable clinical benefit independent of medication use.
