Common transient side effects include localized swelling, erythema, tenderness, and bruising at the injection site—typically resolving within a few days. More clinically significant complications may arise, particularly with improper technique or suboptimal product selection. These include vascular compromise—such as arterial occlusion—which can lead to skin necrosis, ulceration, or even permanent tissue loss if not recognized and managed promptly.
Other reported complications include filler migration, asymmetry, palpable nodules, and delayed-onset inflammatory reactions such as granuloma formation. Rare but serious systemic events—including vision loss due to retrobulbar artery embolism—have been documented in the literature following nasal filler injections, underscoring the anatomical complexity of the nasal region and its rich collateral vasculature.
Patient selection, thorough anatomical knowledge, use of cannulas over needles where appropriate, aspiration before injection, and immediate access to hyaluronidase—all remain critical components of risk mitigation. Patients should be counseled preoperatively about both expected outcomes and realistic complication profiles to support informed consent and shared decision-making.
