Hyaluronic acid is a glycosaminoglycan naturally present in skin, joints, and connective tissues—its primary physiological roles include hydration, lubrication, and structural support. When administered as a dermal filler, HA attracts and binds water molecules, restoring volume, smoothing wrinkles, and enhancing facial contours.
When performed by a qualified, board-certified dermatologist or plastic surgeon using FDA- or EMA-approved products, HA filler injections are considered low-risk with an excellent safety record. Adverse events are typically mild and transient—such as localized swelling, bruising, tenderness, or erythema—and resolve spontaneously within days to a week. Serious complications—including vascular occlusion, tissue necrosis, or blindness—are exceedingly rare but underscore the critical importance of anatomical expertise, proper injection technique, and immediate recognition of warning signs like sudden pallor, sharp pain, or blanching.
Unlike permanent implants or synthetic polymers, HA fillers are biodegradable and metabolized naturally by tissue hyaluronidases over 6–18 months, depending on product formulation, injection depth, and individual metabolism. Their effects are also reversible: if needed, hyaluronidase—an enzyme that enzymatically cleaves HA—can be injected to rapidly dissolve the filler, offering a significant safety advantage over non-hydrolyzable agents.
Patient selection remains essential. Individuals with active skin infections at the treatment site, uncontrolled autoimmune conditions, or a history of severe allergic reactions to HA should defer treatment. While systemic toxicity is not associated with standard aesthetic doses, off-label use—such as large-volume injections in non-FDA-cleared areas or repeated high-dose administration—may increase complication risks and is not supported by robust clinical evidence.
