Do I Have a Deep Bite?

Deep overbite is a common dental occlusion issue characterized by excessive vertical overlap of the upper front teeth over the lower front teeth. In a normal bite, the upper incisors typically cover about 2–4 mm—or roughly 20–30%—of the clinical crown height of the lower incisors. A deep overbite is clinically diagnosed when this overlap exceeds 4–5 mm or when more than 50% of the lower incisor crown is obscured by the upper incisors.

This condition may arise from skeletal factors—such as a vertically deficient mandible or excessive maxillary alveolar development—or dental causes, including overeruption of anterior teeth, undereruption of posterior teeth, or a combination of both. It can also be associated with habits like chronic thumb-sucking or tongue thrusting during childhood development.

Clinically, deep overbite may contribute to premature wear of the lower incisors, gingival trauma (especially on the palatal gingiva of the upper incisors or labial gingiva of the lower incisors), reduced vertical dimension of occlusion, and increased risk of chipping or fracture of anterior teeth. Some patients report discomfort during chewing or difficulty achieving stable occlusal contact.

Accurate diagnosis requires comprehensive evaluation—including clinical examination, study models, and cephalometric radiographs—to differentiate between dental and skeletal etiologies. Treatment options vary based on severity and underlying cause, and may include orthodontic intervention (e.g., intrusion of upper incisors or extrusion of posterior teeth), functional appliances in growing patients, or in select adult cases, orthognathic surgery combined with fixed appliance therapy.
