Injecting hyaluronic acid (HA) fillers into the forehead carries significant and potentially serious risks that clinicians and patients must carefully weigh before proceeding. While HA fillers are widely used for facial rejuvenation, the forehead represents a high-risk anatomical area due to its complex vascular architecture—including terminal branches of the supraorbital and supratrochlear arteries—and proximity to the ophthalmic artery and cavernous sinus.
One of the most severe complications is vascular occlusion, which can lead to skin necrosis, ulceration, or permanent scarring. In rare but catastrophic cases, retrograde embolization through anastomotic channels may result in central retinal artery occlusion—causing sudden, irreversible vision loss—or even cerebral infarction. These events, though uncommon, underscore the necessity of meticulous injection technique, including aspiration before injection, use of low-volume, slow bolus delivery, and avoidance of deep subperiosteal placement.
Additional concerns include unnatural contour irregularities, such as visible ridges or asymmetry, particularly when excessive volume is placed over the frontal bone. Transient adverse effects like edema, ecchymosis, and palpable nodules are relatively common; however, persistent lumps, granulomatous inflammation, or delayed-onset swelling may indicate improper product selection, suboptimal placement, or immune-mediated reactions.
Given these risks, many dermatologists and plastic surgeons advocate for conservative or non-invasive alternatives—such as neuromodulators for dynamic horizontal lines or surgical brow lifts for structural ptosis—rather than HA filler augmentation of the forehead. When fillers are deemed appropriate, they should be administered only by experienced, board-certified providers with advanced training in facial vascular anatomy and emergency management of filler complications.
