Placental growth factor (PlGF) is a key angiogenic protein produced primarily by the syncytiotrophoblast layer of the placenta. Clinically, quantitative measurement of PlGF in maternal serum—typically performed between 18 and 36 weeks’ gestation—serves as a valuable biomarker for assessing placental health and identifying pregnancies at increased risk for preeclampsia, fetal growth restriction (FGR), and other placenta-mediated disorders.
Low circulating PlGF levels reflect impaired trophoblast invasion and inadequate placental vascular remodeling—hallmarks of early-onset preeclampsia and severe FGR. When integrated into multi-parameter algorithms—such as the FMF (Fetal Medicine Foundation) algorithm, which combines PlGF with maternal factors, mean arterial pressure, and uterine artery Doppler pulsatility index—PlGF significantly improves first- and second-trimester risk stratification. A PlGF concentration below the 5th percentile for gestational age has high negative predictive value (>99%) for ruling out preeclampsia requiring delivery within 14 days, supporting timely clinical decision-making and reducing unnecessary hospital admissions.
Importantly, serial PlGF measurements offer dynamic insight: a rising trend suggests improving placental function, whereas persistently low or declining values warrant intensified surveillance—including more frequent blood pressure monitoring, serial fetal growth ultrasounds, and consideration of antenatal corticosteroids or early delivery depending on gestational age and clinical context. While PlGF testing does not replace clinical assessment, it enhances objectivity in risk evaluation and supports individualized management pathways for high-risk pregnancies.
