While modern clinical evidence remains limited and largely preliminary, historical practice and emerging research suggest several potential physiological effects associated with moxibustion.
One well-documented application is in obstetrics: randomized controlled trials have demonstrated that moxibustion applied to the BL67 (Zhiyin) acupoint—located on the lateral aspect of the little toe—may increase the likelihood of cephalic version in women with breech presentation during the third trimester. The proposed mechanism involves thermal stimulation that may influence uterine muscle tone or fetal motility, though precise neurophysiological pathways remain under investigation.
Moxibustion is also frequently employed for musculoskeletal conditions, including chronic low back pain and osteoarthritis. Some clinical studies report modest improvements in pain scores and functional mobility compared to sham controls, potentially mediated through localized vasodilation, modulation of inflammatory cytokines such as IL-1β and TNF-α, and activation of endogenous opioid pathways. However, effect sizes tend to be small, and high-quality, large-scale trials are still needed to confirm efficacy and standardize protocols.
Additionally, practitioners sometimes use moxibustion to support digestive health—particularly in cases of functional dyspepsia or diarrhea-predominant irritable bowel syndrome—based on traditional diagnostic frameworks emphasizing “spleen yang deficiency.” While plausible mechanisms include autonomic nervous system modulation and enhanced gastrointestinal motility, robust clinical data supporting these indications are currently lacking.
It is important to emphasize that moxibustion is not without risk. Contraindications include fever, local skin infection or lesions, impaired sensation (e.g., in diabetic neuropathy), and pregnancy outside of carefully indicated scenarios such as breech version. Adverse events—though rare—may include burns, allergic contact dermatitis, or inhalation-related respiratory irritation from smoke exposure.
