The underlying mechanisms are multifactorial. Vasovagal responses—triggered by anxiety, pain perception, or autonomic nervous system sensitivity—are the most frequent cause. Needle stimulation, particularly at sensitive points such as those on the neck, scalp, or abdomen, may provoke a reflexive drop in heart rate and blood pressure, leading to lightheadedness and nausea. Other contributing factors include hypoglycemia (especially if the patient has not eaten prior to treatment), dehydration, fatigue, or preexisting vestibular or cardiovascular conditions.

While generally benign, these symptoms warrant clinical attention. Practitioners should screen for contraindications—including uncontrolled hypertension, recent myocardial infarction, or severe arrhythmias—and assess patients’ baseline autonomic stability before treatment. Proper patient positioning (supine or semi-reclined), gradual needle insertion, and post-treatment rest periods help mitigate risk. Patients should be advised to eat a light meal and hydrate adequately before sessions.

Although rare, persistent or worsening dizziness or nausea—particularly when accompanied by visual disturbance, speech difficulty, or limb weakness—requires immediate evaluation to rule out serious neurological or cardiovascular events unrelated to acupuncture. In most cases, however, symptom resolution is rapid and complete, with no long-term sequelae.
