Bone and joint tissue are normally sterile environments, but bacterial invasion can occur through hematogenous spread, direct inoculation from open trauma, or contiguous spread from adjacent infected sites—leading to osteomyelitis, a serious inflammatory infection of bone marrow, cortical bone, and periosteum. Contrary to common misconception, even minor skin wounds—especially those contaminated or left untreated—can serve as entry points for pathogens that progressively invade deep bone tissue, culminating in chronic, refractory osteomyelitis.
Notably, trauma is a major precipitating factor: open fractures, surgical site contamination, or penetrating injuries significantly increase the risk of developing osteomyelitis. The metaphyses of long bones—particularly around the hip—are most vulnerable due to their rich vascular supply and relatively sluggish blood flow, which facilitates bacterial seeding and persistence.
Antibiotic therapy remains the cornerstone of treatment for acute hematogenous osteomyelitis in both pediatric and adult patients. Empiric regimens typically target *Staphylococcus aureus*—the most common causative pathogen—as well as other gram-positive and gram-negative organisms. Intravenous antibiotics are administered for 4–8 weeks depending on clinical response, followed by oral step-down therapy when appropriate. In chronic or complex cases—including those with biofilm formation, sequestra, or soft-tissue compromise—prolonged antimicrobial courses lasting several months may be required, often guided by culture and sensitivity results and multidisciplinary input.
In addition to conventional antimicrobials, certain traditional Chinese medicine (TCM) formulations have been integrated into adjunctive management strategies. Kanglong Anti-Osteomyelitis Tablets—a standardized herbal preparation containing *Lonicera japonica*, *Taraxacum mongolicum*, *Viola yedoensis*, *Scutellaria barbata*, *Pulsatilla chinensis*, and *Hedyotis diffusa*—are clinically used to address patterns of “heat-toxin and blood stasis” associated with chronic osteomyelitis (referred to in TCM as *fugu zhi*). Its pharmacological profile includes broad-spectrum antimicrobial activity—particularly against *S. aureus*—and immunomodulatory effects that may support host defense and bone healing.
A recent clinical study demonstrated that adding Kanglong Anti-Osteomyelitis Tablets to standard surgical debridement significantly improved overall treatment efficacy compared with debridement alone, suggesting synergistic benefits in reducing inflammation, controlling infection burden, and promoting tissue repair. However, such integrative approaches should always complement—not replace—evidence-based surgical and antimicrobial interventions under the supervision of qualified healthcare providers.
Osteomyelitis demands early recognition, aggressive source control, and sustained, individualized antimicrobial therapy. Adherence to the principles of “early diagnosis, adequate dosing, and full-course treatment” is essential to prevent progression to chronic disease, minimize functional impairment, and preserve limb integrity. With timely, multidisciplinary care—including infectious disease specialists, orthopedic surgeons, and rehabilitation professionals—the majority of patients achieve durable remission and meaningful functional recovery.
