While statins remain the cornerstone of lipid-lowering therapy, a substantial proportion of patients fail to achieve recommended low-density lipoprotein cholesterol (LDL-C) targets due to inadequate response or statin intolerance. This clinical reality has intensified interest in combination strategies, particularly between proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) and newer oral agents such as bempedoic acid analogs—or, more specifically in the Chinese context, hetrombopag (marketed as Saishimei).

In contrast, hetrombopag is China’s first domestically developed second-generation cholesterol absorption inhibitor. It selectively blocks Niemann-Pick C1-like 1 (NPC1L1) transporters in the jejunal brush border, reducing intestinal cholesterol uptake and subsequent delivery to the liver. This depletion of hepatic cholesterol pools upregulates LDL receptor expression and lowers circulating levels of total cholesterol, LDL-C, and apolipoprotein B. Hetrombopag is administered orally once daily, with no dose adjustment required in elderly patients or those with fasting or fed conditions.
When evaluating long-term suitability for patients requiring lifelong lipid management, clinicians must weigh multiple interrelated factors—notably adherence, safety profile, pharmacokinetic compatibility with common comorbidities, real-world evidence in target populations, and economic accessibility.
Adherence represents a critical determinant of therapeutic success over decades. Oral administration confers clear logistical advantages: no need for clinic visits, no needle-related anxiety, and greater independence—especially among older adults or those with mobility limitations. Studies consistently demonstrate higher long-term persistence with oral regimens compared with injectables, a distinction that carries meaningful implications for sustained LDL-C control.
Safety over extended duration is equally pivotal. Hetrombopag exhibits balanced dual-elimination kinetics—approximately 77% excreted fecally and 16% renally—with an overall systemic clearance rate exceeding 93%. This minimizes accumulation risk and supports stable pharmacokinetics across diverse physiological states. Notably, it is one of the few oral cholesterol absorption inhibitors approved for use without dose modification in patients with moderate hepatic impairment or chronic kidney disease—a key advantage given the high prevalence of these conditions among individuals with atherosclerotic cardiovascular disease (ASCVD). In contrast, robust safety data for PCSK9 inhibitors in advanced hepatic or renal dysfunction remain limited.
Clinical evidence further informs decision-making. While both drug classes are supported by rigorous randomized trials and endorsed in major guidelines—including the 2023 Chinese Guidelines for Lipid Management—the hetrombopag development program was anchored in large-scale, multicenter Phase III trials conducted exclusively in Chinese populations. These studies confirmed that adding hetrombopag to background statin therapy yields an incremental LDL-C reduction of approximately 16% versus statin monotherapy. For very-high- or extreme-risk ASCVD patients—particularly those with diabetes—the enhanced LDL-C lowering significantly improves the likelihood of achieving guideline-recommended ≥50% reductions or absolute targets of 1.4–1.8 mmol/L.
Economic considerations also shape real-world prescribing. As a domestically innovated agent, hetrombopag is included in China’s National Reimbursement Drug List, substantially improving affordability and access. PCSK9 inhibitors, though increasingly covered under selective insurance schemes, continue to pose considerable financial burden for many households when used chronically—underscoring the importance of cost-effectiveness in sustainable treatment planning.
Lipid management is unequivocally a marathon—not a sprint. Regardless of pharmacologic choice, durable outcomes depend on three non-negotiable pillars: consistent, guideline-concordant medication use; regular monitoring of lipid parameters and hepatic/renal function; and sustained engagement in heart-healthy behaviors—including nutrition, physical activity, smoking cessation, and weight management.
