As spring brings fluctuating temperatures and heightened respiratory vulnerability, clinicians are urging greater awareness of such peripheral manifestations, particularly those linked to chronic hypoxia or systemic inflammation originating in the lungs.
1. Digital clubbing: A telltale sign of chronic hypoxia
Digital clubbing is characterized by bulbous enlargement of the distal phalanges and a loss of the normal angle between the nail plate and proximal nail fold (Lovibond angle > 180°). Unlike typical tapering digits, affected fingers exhibit symmetric, soft-tissue swelling at the fingertips, often accompanied by increased nail curvature and a spongy, “drumstick” appearance. This morphological change reflects chronic tissue hypoxia—most commonly driven by impaired gas exchange in conditions such as interstitial lung disease, non-small cell lung cancer, bronchiectasis, or cystic fibrosis. In some cases, it reflects hypertrophic osteoarthropathy (HOA), a paraneoplastic or inflammatory syndrome frequently associated with intrathoracic pathology. They are neither isolated nor incidental—they reflect integrated physiological responses to pulmonary dysfunction. Early recognition allows for targeted diagnostic workup, including pulse oximetry, high-resolution CT chest imaging, and spirometry. Preventive strategies—including smoking cessation, indoor air quality optimization, regular aerobic exercise to enhance ventilatory efficiency, and vaccination against influenza and pneumococcus—remain foundational to preserving long-term pulmonary health.
