Sudden upper abdominal pain can be deeply misleading—what feels like indigestion or gastritis may, in fact, be an acute myocardial infarction (AMI) masquerading as gastrointestinal distress. A recent case involving a middle-aged man who self-treated presumed gastritis with over-the-counter antacids nearly resulted in fatal delay of care—a stark reminder that cardiac ischemia often presents atypically, especially in men over 40 and individuals with cardiovascular risk factors.
Location: Where the Pain Lies—and Lies Again
Gastritis typically causes localized, cramp-like discomfort centered in the epigastric region—the area just below the xiphoid process and above the umbilicus. The pain often intensifies with gastric distension (e.g., after eating spicy or fatty foods), changes with posture, and may be reproducible on palpation.
In contrast, myocardial ischemia frequently manifests with referred pain that defies classic anatomical expectations. While substernal pressure or tightness remains the hallmark, up to 30% of patients report pain radiating to the left shoulder, jaw, neck, upper back, or even the left hand—particularly the fourth and fifth fingers. This “radiating” pattern reflects shared spinal cord dermatomes between cardiac and somatic structures, not true gastrointestinal pathology.
Associated Symptoms: Clues Beyond the Complaint
Gastritis commonly co-presents with nausea, belching, acid reflux, and occasionally vomiting—symptoms rooted in gastric mucosal irritation and altered motility. Diarrhea or generalized abdominal tenderness further supports a gastrointestinal etiology.
Conversely, AMI often triggers autonomic responses indicative of systemic sympathetic activation and impending hemodynamic compromise: profuse diaphoresis (especially cold, clammy sweat), pallor, lightheadedness, near-syncope, or unexplained dyspnea. Left-hand paresthesia—particularly involving the ulnar distribution—is a recognized but underappreciated red flag, reflecting neural referral patterns from the inferior myocardial wall.
Temporal Pattern: Acute Onset Versus Chronicity
Gastritis tends toward recurrent, intermittent symptoms—worsening with dietary triggers, fasting, or NSAID use—and may persist for days without dramatic escalation. In such cases, outpatient evaluation with endoscopy or H. pylori testing is appropriate.
By contrast, ST-elevation or non-ST-elevation myocardial infarction typically presents with abrupt, severe, unremitting discomfort lasting more than 15 minutes—often occurring at rest, upon awakening, or following exertion. Notably, exertional epigastric pain—such as sudden upper abdominal distress during morning jogging—should prompt immediate cessation of activity and urgent cardiac assessment, not antacid administration.
When faced with new-onset upper abdominal pain, clinicians and patients alike must apply a diagnostic triad: evaluate pain location and radiation, assess associated autonomic and neurological symptoms, and scrutinize temporal dynamics. Self-treatment with antacids or proton-pump inhibitors carries no risk in confirmed gastritis—but delays definitive management in acute coronary syndrome, where every minute counts.
