These symptoms are not merely signs of fatigue or poor sleep hygiene; they may reflect underlying arterial or venous compromise—often an early manifestation of atherosclerosis, stenosis, or thrombosis. Vascular pathology rarely announces itself with dramatic flair. Instead, it whispers through nighttime physiology—when cardiac output drops, peripheral resistance shifts, and autonomic regulation changes—making sleep a uniquely revealing diagnostic window.
Nighttime chest discomfort and dyspnea warrant urgent clinical attention. Patients may report orthopnea—increasing breathlessness upon lying flat—requiring them to sit upright to relieve pressure-like chest tightness. This occurs because supine positioning elevates venous return, exacerbating pulmonary congestion when left ventricular function is impaired by coronary artery disease. More alarmingly, paroxysmal nocturnal dyspnea—abrupt awakening with suffocation, gasping, and anxiety—signals decompensated heart failure, frequently rooted in chronic ischemic cardiomyopathy. These three symptom clusters—nocturnal cardiorespiratory distress, limb ischemia, and cerebral hypoperfusion—are not isolated events. They represent interconnected pathophysiological consequences of systemic atherosclerosis. Sleep provides a natural stress test: hemodynamic fluctuations, vagal dominance, and altered coagulation profiles during rest unmask vulnerabilities that remain silent during daytime activity. Clinicians should view recurrent, unexplained nocturnal symptoms as potential harbingers of cardiovascular morbidity—not just markers of poor sleep quality. Diagnostic evaluation must include noninvasive vascular imaging: carotid Doppler ultrasound, ankle-brachial index testing, transthoracic echocardiography, and, when indicated, CT angiography or MR angiography. Lifestyle intervention—smoking cessation, hypertension and diabetes control, lipid-lowering therapy, and supervised exercise—is foundational. Delaying assessment risks progression to myocardial infarction, limb amputation, or stroke.
