Sleep-related eating disorder (SRED), a parasomnia characterized by recurrent episodes of involuntary eating during partial arousal from sleep, must also be distinguished—especially when patients have no memory of the behavior or consume unusual or non-nutritive items.
Lifestyle and behavioral contributors are frequently implicated: skipping dinner, consuming low-protein or high-glycemic-index meals late in the evening, excessive caffeine or alcohol intake close to bedtime, or chronic sleep deprivation (which disrupts leptin and ghrelin regulation, increasing appetite signaling). Shift work and circadian misalignment can further impair metabolic homeostasis and amplify nocturnal hunger cues.
A thorough assessment should include a detailed dietary and sleep history, review of medications (especially hypoglycemic agents, corticosteroids, or psychotropics), physical examination, and targeted investigations—such as fasting and 72-hour supervised fasting plasma glucose and insulin levels, HbA1c, thyroid function tests, and cortisol rhythm assessment if indicated. Management is tailored to the underlying cause but often begins with dietary modification (e.g., balanced evening meals containing adequate protein and complex carbohydrates), consistent sleep hygiene, and avoidance of late-night stimulants.
