What does it mean when the fetal heart rate is high?
Fetal tachycardia refers to a sustained fetal heart rate above 160 beats per minute (bpm) for more than 10 minutes. It is a common finding during antenatal monitoring and may reflect a range of physiological or pathological conditions. Transient episodes—often lasting less than 10 minutes—can occur in response to fetal movement, maternal fever, maternal anxiety, or mild sympathetic stimulation and are typically benign. However, persistent tachycardia warrants further evaluation.
Common non-pathological causes include maternal factors such as fever (especially with infections like influenza or urinary tract infection), hyperthyroidism, anemia, dehydration, or stimulant use (e.g., caffeine, certain medications). Fetal causes may include early gestational age (normal higher baseline rates in the second trimester), fetal hypoxia, infection (e.g., congenital viral infections such as parvovirus B19 or cytomegalovirus), arrhythmias (e.g., supraventricular tachycardia), or structural cardiac anomalies.
Clinical assessment should include maternal vital signs, review of medications and recent illness, and detailed fetal evaluation via ultrasound with Doppler assessment—including biophysical profile, amniotic fluid index, and targeted echocardiography if arrhythmia or structural abnormality is suspected. Continuous electronic fetal monitoring may be indicated in labor or for serial outpatient assessment.
Management depends on the underlying cause: treating maternal fever or infection, correcting electrolyte imbalances or anemia, discontinuing stimulants, or initiating antiarrhythmic therapy (e.g., digoxin or flecainide) for sustained fetal supraventricular tachycardia. Referral to a maternal-fetal medicine specialist and pediatric cardiology is recommended when tachycardia is persistent, associated with hydrops fetalis, or accompanied by other concerning ultrasound findings.
