What could cause pimples to appear on a woman’s genital area?

When women develop small, raised bumps or pimples in the genital area—commonly referred to as “genital acne”—it’s important to recognize that these lesions are rarely true acne (i.e., comedones resulting from pilosebaceous unit inflammation). Instead, they often represent other benign, non-acne conditions. Common causes include folliculitis (inflammation or infection of hair follicles, frequently due to *Staphylococcus aureus* or pseudomonas after hot tub exposure), molluscum contagiosum (a viral infection causing smooth, pearly, umbilicated papules),Fordyce spots (ectopic sebaceous glands appearing as painless, flesh-colored or yellowish bumps), or vestibular papillomatosis (a normal anatomical variant characterized by symmetric, soft, filiform or dome-shaped papillae on the inner labia minora). Less commonly, such lesions may signal sexually transmitted infections—including genital warts (caused by human papillomavirus), herpes simplex virus (HSV) presenting as grouped, tender vesicles or ulcers—or, rarely, early-stage lichen planus or hidradenitis suppurativa in intertriginous areas. Any new, persistent, painful, ulcerated, bleeding, or rapidly changing lesion warrants prompt clinical evaluation by a gynecologist or dermatologist for accurate diagnosis—often requiring physical examination, dermoscopy, or targeted testing (e.g., PCR for HSV/HPV, KOH prep, or biopsy). Self-treatment with acne products (e.g., benzoyl peroxide or retinoids) is discouraged, as these can irritate delicate genital skin and delay appropriate management.
