What are the risks of administering growth hormone to children?
Growth hormone (GH) therapy in children is a well-established, FDA-approved treatment for specific medical conditions, including growth hormone deficiency (GHD), Turner syndrome, chronic kidney disease–related short stature, Prader-Willi syndrome, and idiopathic short stature (ISS) under strict criteria. When prescribed appropriately—based on thorough diagnostic evaluation, confirmed biochemical and clinical evidence of GH insufficiency or an approved indication—and administered under close pediatric endocrinology supervision, GH therapy is generally safe and effective.
However, like all pharmacologic interventions, GH carries potential risks and adverse effects. Common, usually mild and reversible effects include fluid retention (e.g., peripheral edema, arthralgia, carpal tunnel symptoms), transient hyperglycemia, and injection-site reactions. More serious but rare complications may include intracranial hypertension (pseudotumor cerebri), slipped capital femoral epiphysis (SCFE), and progression of preexisting scoliosis. There is no conclusive evidence that GH therapy increases long-term cancer risk in children without prior malignancy; however, GH is contraindicated in patients with active neoplasms or those who have not completed anticancer therapy.
Crucially, GH is not indicated for normal variant short stature, athletic enhancement, or cosmetic height improvement. Inappropriate use—such as off-label prescribing without rigorous diagnostic confirmation or inadequate monitoring—can lead to unnecessary exposure, financial burden, psychological harm, and avoidable complications. Therefore, decisions about GH therapy must be individualized, evidence-based, and guided by comprehensive assessment including auxology, bone age radiography, provocative GH testing (when indicated), IGF-1 and IGFBP-3 levels, and exclusion of alternative causes of short stature.
Families should engage in shared decision-making with a qualified pediatric endocrinologist, weighing realistic expectations, treatment duration (often several years), monitoring requirements (e.g., regular height velocity assessments, thyroid function, glucose metabolism, and skeletal surveillance), and psychosocial considerations. Ongoing safety surveillance remains essential throughout therapy and into transition to adult care when appropriate.
