If you’ve noticed a hard lump near your testicle, it’s important to take this seriously and seek prompt medical evaluation. A variety of conditions can cause such a finding—including benign causes like epididymal cysts, spermatoceles, or varicoceles—but also potentially serious conditions such as testicular cancer, epididymitis, orchitis, or inguinal hernias. Testicular cancer, while relatively rare, most commonly presents as a painless, firm, or hard mass within or adjacent to the testicle, often described as feeling like a “pebble” or “marble.” Other red flags include swelling, heaviness in the scrotum, dull ache in the lower abdomen or groin, or sudden fluid accumulation in the scrotum.

A healthcare provider will perform a thorough physical exam—including transillumination and palpation—and almost always order a scrotal ultrasound, which is the gold-standard imaging modality for evaluating intrascrotal and paratesticular masses. Blood tests for tumor markers (e.g., alpha-fetoprotein [AFP], beta-human chorionic gonadotropin [β-hCG], and lactate dehydrogenase [LDH]) may also be indicated if malignancy is suspected.

Early diagnosis significantly improves outcomes, especially for treatable conditions like testicular cancer, which has an excellent prognosis when detected and managed at an early stage.
