While the term “heart pain” may immediately evoke concerns about cardiac disease, this sensation can arise from a broad spectrum of conditions spanning multiple organ systems.
Cardiovascular causes remain among the most critical to consider. Stable angina pectoris typically presents as pressure, tightness, or squeezing in the chest, often triggered by exertion or emotional stress and relieved by rest or nitroglycerin. In contrast, myocardial infarction may manifest with more intense, persistent pain—though some patients, particularly older adults, women, and those with diabetes, report only mild or atypical discomfort, including subtle substernal ache without classic radiation or diaphoresis. Other cardiac contributors include pericarditis (often sharp and positional), myocarditis, and aortic dissection (characterized by sudden, tearing pain).
Non-cardiac etiologies are equally prevalent. Gastroesophageal reflux disease (GERD) frequently mimics cardiac pain, producing a burning or dull substernal sensation exacerbated by meals, recumbency, or bending. Esophageal spasm or motility disorders may cause similar intermittent, non-exertional discomfort. Musculoskeletal sources—including costochondritis, Tietze syndrome, or myofascial strain—often yield localized tenderness and pain reproducible with palpation or movement.
Pulmonary conditions such as pleuritis, pulmonary embolism, or early pneumonia can also refer pain to the anterior chest wall. Anxiety and panic disorders commonly present with vague chest tightness or aching, frequently accompanied by hyperventilation, palpitations, or a sense of impending doom—yet these diagnoses should only be assigned after thorough exclusion of organic pathology.
Because the differential diagnosis ranges from benign and self-limited to life-threatening, clinical assessment must prioritize risk stratification. Key red flags include pain associated with dyspnea, diaphoresis, nausea, syncope, radiation to the jaw or left arm, or new-onset symptoms in individuals with cardiovascular risk factors. A comprehensive evaluation—including electrocardiography, cardiac biomarkers, imaging, and targeted testing based on clinical suspicion—is essential to guide appropriate management and avoid diagnostic delay.
