Viral upper respiratory infections remain the most frequent cause—especially in preschool-aged children, who may experience six to eight such episodes annually. In these cases, cough may persist for two to three weeks after the fever resolves, a phenomenon known as post-viral cough. However, recurrent febrile episodes with cough occurring more than six to eight times per year—or lasting longer than four weeks—warrant further evaluation.
Chronic or recurrent lower respiratory tract involvement raises suspicion for conditions such as asthma, allergic rhinitis with postnasal drip, gastroesophageal reflux disease (GERD), or chronic sinusitis. Less commonly, persistent symptoms may reflect immunodeficiency disorders, cystic fibrosis, primary ciliary dyskinesia, or structural airway abnormalities like tracheobronchomalacia or bronchiectasis.
Red flags that necessitate prompt pediatric referral include failure to thrive, digital clubbing, hemoptysis, nocturnal cough disrupting sleep, exertional dyspnea, or recurrent pneumonia. Diagnostic workup is tailored to clinical context but may include spirometry, fractional exhaled nitric oxide (FeNO) testing, chest radiography or high-resolution CT, sweat chloride testing, immunoglobulin panels, or flexible bronchoscopy.
Management hinges on accurate diagnosis: supportive care for viral illness, inhaled corticosteroids for asthma, nasal corticosteroids for allergic rhinitis, acid suppression and lifestyle modification for GERD, and targeted antimicrobial therapy when bacterial infection is confirmed. Empiric antibiotic use without evidence of bacterial superinfection is discouraged due to risks of resistance and adverse effects.
Clinicians should adopt a systematic, stepwise approach—balancing vigilance for serious pathology with recognition of the benign, self-limited nature of many childhood respiratory illnesses.
