Postoperative abdominal pain following an appendectomy is a common concern among patients recovering from acute appendicitis. While mild, transient discomfort at the incision site or in the lower right quadrant is expected during the first few days after surgery, persistent, worsening, or new-onset pain warrants clinical evaluation.
Several potential causes must be considered. Surgical site infection—characterized by increasing tenderness, erythema, warmth, swelling, or purulent drainage—is one of the most frequent complications. Intra-abdominal abscess formation, particularly in cases of perforated or gangrenous appendicitis, may present with fever, leukocytosis, and localized or diffuse abdominal pain, often accompanied by ileus or systemic inflammatory signs.
Other possibilities include adhesive small bowel obstruction, especially in patients with prior abdominal surgeries; residual inflammation or seroma at the stump site; or, less commonly, stump appendicitis—recurrent inflammation of retained appendiceal tissue. Rarely, non-surgical conditions such as Crohn’s disease, mesenteric lymphadenitis, or gynecologic pathology (e.g., ovarian torsion or pelvic inflammatory disease) may mimic or coexist with post-appendectomy symptoms.
Patients experiencing fever, nausea, vomiting, rebound tenderness, guarding, or pain that intensifies with movement or coughing should seek prompt medical assessment. Diagnostic workup typically includes a thorough physical examination, complete blood count, C-reactive protein, and imaging—most often contrast-enhanced CT of the abdomen and pelvis—to identify fluid collections, abscesses, or structural abnormalities.
Management depends on the underlying cause: antibiotics and percutaneous drainage for abscesses, surgical intervention for complications such as bowel obstruction or recurrent infection, and supportive care for uncomplicated postoperative discomfort. Early recognition and targeted intervention significantly reduce morbidity and improve recovery outcomes.
