While minor spacing changes may occur naturally with age, progressive or sudden increases in gap size often reflect more serious conditions requiring professional evaluation.
One of the most frequent causes is periodontitis, a chronic inflammatory disease affecting the supporting structures of the teeth. As inflammation advances, it triggers destruction of the periodontal ligament and alveolar bone—key components that anchor teeth in place. This loss of structural support leads to tooth mobility and drifting, particularly in the anterior region, resulting in visible separation between teeth.
Gingival recession—often linked to aggressive brushing, orthodontic movement, or thin biotype gums—can also create the illusion of larger gaps. When the gum tissue recedes, previously covered root surfaces become exposed, increasing the apparent distance between adjacent teeth at the gingival margin.
Other contributing factors include long-standing occlusal trauma (e.g., from bruxism or malocclusion), which may accelerate tooth migration; untreated dental caries or failed restorations leading to proximal tooth structure loss; and, less commonly, systemic conditions such as uncontrolled diabetes or osteoporosis that impair bone metabolism and periodontal stability.
Patients noticing new or worsening interdental spacing should seek comprehensive evaluation by a dentist or periodontist. Diagnostic workup typically includes clinical periodontal probing, radiographic assessment (e.g., bitewing or panoramic radiographs) to evaluate bone levels, and occlusal analysis. Early intervention—ranging from non-surgical periodontal therapy and occlusal adjustment to surgical regeneration or prosthetic rehabilitation—can halt progression and preserve dentition.
