Coronary artery disease (CAD) and heart disease are terms often used interchangeably in everyday conversation—but medically, they represent distinct concepts. Heart disease is an umbrella term encompassing a broad range of conditions that affect the heart’s structure or function. These include arrhythmias, valvular heart disease, cardiomyopathies, congenital heart defects, heart failure, and infections such as myocarditis or endocarditis.
In contrast, coronary artery disease specifically refers to the narrowing or blockage of the coronary arteries—the vessels that supply oxygen-rich blood to the heart muscle itself. This obstruction most commonly results from atherosclerosis: the progressive buildup of cholesterol-rich plaques within the arterial walls. As plaques grow, they restrict blood flow, potentially leading to angina (chest pain or discomfort), myocardial infarction (heart attack), or sudden cardiac death.
While CAD is the most prevalent type of ischemic heart disease—and the leading cause of cardiovascular mortality worldwide—it is only one subset of heart disease. Not all individuals with heart disease have CAD; for example, a patient with mitral valve regurgitation or dilated cardiomyopathy may have significant cardiac impairment without any evidence of coronary artery stenosis. Conversely, many patients diagnosed with CAD do not yet exhibit overt symptoms of heart failure or structural heart disease—though untreated CAD significantly increases long-term risk for those complications.
Clinically, distinguishing between these categories guides diagnostic strategy and therapeutic priorities. Evaluating suspected CAD typically involves noninvasive testing such as stress echocardiography or coronary CT angiography, followed by invasive coronary angiography if indicated. Management emphasizes risk factor modification (e.g., lipid-lowering therapy, hypertension control, smoking cessation), antiplatelet agents, and revascularization when appropriate. Broader heart disease diagnoses may require different modalities—such as transthoracic echocardiography for valvular assessment or cardiac MRI for myocardial tissue characterization—and tailored interventions ranging from device-based therapies to surgical correction.
